DIAGNOSING RECTILE DYSFUNCTION

by your doctor.
They will talk to you about your situation and may carry out a physical examination.
The sexual history must include (when available) information about sexual orientation, previous and current sexual relationships, current emotional status, onset and duration of the erectile problem, and previous consultations and treatments.
A detailed description should be made of the rigidity and duration of both sexually-stimulated and morning erections and of problems with sexual desire, arousal, ejaculation, and orgasm. Validated psychometric questionnaires, such as the International Index for Erectile Function or its short version Sexual Health Inventory for Men, help to assess the different sexual function domains, as well as the impact of a specific treatment modality.
You may be asked about:
Whether you have been able to ejaculate or orgasm
Your libido (your level of sexual desire)
Your previous and current sexual relationships
Whether you can get any degree of erection with your partner, on your Own or when you wake up in the morning
What your sexual orientation is
How long you have been experiencing erectile dysfunction
Erectile dysfunction that only occurs when you are attempting to have sex may suggest an underlying psychological (mental) cause.
Assessing your cardiovascular health
Your doctor may assess your cardiovascular health.
Physical examinations and laboratory testing
A physical examination of your penis may be carried out to rule out anatomical causes (conditions that affect the physical structure of your penis).
Every patient must be given a physical examination focused on the genitourinary, endocrine, vascular, and neurological systems.
Laboratory testing must be tailored to the patient’s complaints and risk factors. Patients may need a fasting
blood glucose or HbA1c and lipid profile if not recently assessed. Hormonal tests include an early morning total testosterone, bioavailable or free testosterone.
Specialised diagnostic tests
This might be the case if you are unusually young to be experiencing erectile dysfunction as it's rare in men under 40 years of age.
Duplex ultrasound of the penis
An intracavernous injection test involves injecting a man-made (synthetic) hormone into your penis to increase the blood flow. This helps assess any abnormalities in your penis and plan surgery.
A peak systolic blood flow > 30 cm/s, an end-diastolic velocity of < 3 cm/s and a resistance index > 0.8 are generally considered normal. Further vascular investigation is unnecessary when a Duplex examination is normal.
